Hi @Lisa White (Customer). You will need to go to the app marketplace and enable the perfect portal. You will then go to menu - home - perfect portal dashboard and register. These are general intake forms not customizable to matter types at the moment.
Thanks Lori. I followed the steps and now it is asking me to book a demo (does not give me the option to register). Why do I need to book a demo? Is there an additional charge to use client intake forms?

Once you enable it, open any matter and click on the Intake & Updates button on the LEAP ribbon in the matter, it will be close to the very end.
